Machine learning is proving to be a vital tool for mitigating risks and making decisions in both the public and private sectors. But this predictive capability needs to be translated into practice, especially in high-consequence situations where decisions must be transparent, explainable, secure, and accountable. This review evaluates machine learning frameworks that combine predictive modeling, lifecycle risk management, and governance, and examines how these approaches can be applied to complex decision-making in healthcare, public services, transportation, procurement, finance and other critical areas. The analysis focuses on the change in risk-oriented machine learning from statistical anomaly detection to continuous architectures that are governed by feature stores, model registries, automated deployment, monitoring, audit trails, and risk assessment mechanisms. Special focus is placed on privacy-preserving technologies, access controls, threat modelling, model validation, and practices for accountable deployment in documentation. Some of the key issues are data quality, limited representativeness, algorithmic bias, regulatory complexity, resource limitation, procurement issues, and the requirement for ongoing post-deployment monitoring. Integrated data and AI governance, prioritizing risks, engaging stakeholders, documenting the AI lifecycle, and continuously assessing and improving practices are best practices. Moving forward, real-time analytics, AI, privacy-preserving data sharing, cross-organizational collaboration, and better integration with emerging AI governance standards are among the future directions. In general, the ability to provide trustworthy ML frameworks necessitates a delicate equilibrium between the technical capabilities, management of risk, transparency, security, fairness, and organizational accountability to generate sound and socially just strategic choices.
